Hiroo Hospital requires referrals and appointments as a general rule to facilitate partnerships with local medical institutions. If you have no referral form, you have to pay an Additional Fee for Non-referral Patient First Consultation (1,300 yen) as the Fee for Treatment of the Patients' Choice. Bring a referral form from the medical institution you currently visit.
IMPORTANT:
As of January 1, 2024, the fees will be 7,000 yen for medical patients and 5,000 yen for dental patients. This system has been instituted by the Japanese government to privide appropriate treatment based on a patient's medical conditions.
Appointment for first consultation
The following cases are accepted at the Telephone Appointment Center:
- First consultation in Hiroo Hospital
- First consultation in a department you have never visited
- Consultation in the department again six months or more after the previous consultation
At the first consultation, let us know the following:
- Symptoms
- Name
- Birth date
- Phone number
- Whether or not you have a referral form
- Patient ID card number if you have one
Please note the following:
- You may not be seen at the scheduled time because of such situations the visit of an emergency patient.
- Make appointments as early as possible because it is more difficult to make appointments immediately before the preferred date.
- Keep a record of your telephone appointment, such as the name of department and scheduled time and date.
Making an appointment for further consultation
If you repeatedly visit the same department for your ongoing treatment, you may make the next appointment during the consultation. Ask the doctor or nurses when making an appointment.
If you have to make appointment via telephone for any reason or you have to change the appointment date, contact the Telephone Appointment Center.

Available days for appointments
Appointments are accepted Monday to Friday after the next day.
Appointments for Saturday are not accepted. Appointments for a consultation on the same day are not accepted either.
Change/Cancel
Let us know of any change or cancellation of your appointment by the day before the scheduled date. If you are unable to visit for any reason on the scheduled date, contact the clinic of the department where you have an appointment via the main number.
